Physicals we offer
- Annual physical: a yearly check-up covering your health history, a physical exam, vital signs, and the screenings and vaccines recommended for your age. Most insurance plans cover one preventive visit a year.
- Pre-employment physical: the exam and paperwork your new employer requires. Bring the employer's form, and tell us when you book if any specific tests are needed.
- School physical: the exam and forms needed for school enrollment and activities. Bring the school's form and your immunization records.
- Medicare annual wellness visit: a yearly visit, covered by Medicare, to review your health and build a personalized prevention plan. In your first year on Medicare Part B, you can also have a "Welcome to Medicare" preventive visit.
What a physical includes

- Blood pressure, heart rate, weight and body mass index
- A head-to-toe physical exam
- Blood tests when they are due, such as cholesterol, blood sugar and kidney function
- Cancer screening and vaccines recommended for your age
- Time to ask questions about anything that is bothering you
What to bring
Your insurance card and photo ID, a list of your medications, any forms from your employer or school, your immunization records if you have them, and your completed new patient forms if this is your first visit. If you need to fast before blood tests, we'll tell you when you book.
Insurance and cost
Annual physicals and Medicare wellness visits are covered by most insurance plans, often at no cost to you. Pre-employment and school physicals may not be covered by insurance, so please call us before your visit to ask about cost.
Frequently asked questions
How long does a physical take?
Plan for about 30 to 45 minutes, plus a few minutes for any blood tests.
Is the Medicare wellness visit the same as a physical?
Not exactly. The Medicare annual wellness visit focuses on your prevention plan and health risks and may not include a full head-to-toe exam. Dr. Singh can explain which visit is right for you.
Can you fill out my employer's or school's form?
Yes. Bring the form to your visit and we'll complete it.
Do I need to fast?
Only if you're having certain blood tests, such as cholesterol or blood sugar. We'll let you know when you book.
